Older Person, Frailty & Community Care Centre
Build proactive and responsive care around the person’s home and community, with clear clinical oversight and safe transitions when acuity changes.
High-quality frailty care requires continuity, timely clinical review, rehabilitation, medicines support, social care, care-home partnership and access to escalation when needed. Current NHS England guidance places neighbourhood-level integrated frailty MDTs, urgent community response, virtual wards/hospital at home, intermediate care and palliative support within one connected pathway.
Community hospitals can provide rehabilitation, step-up/step-down care, ongoing medical management and palliative care, but safe models require clarity about senior clinical responsibility, diagnostics, escalation, transfer, medicines, multidisciplinary working and out-of-hours arrangements.
The public centre will teach transferable clinical and governance principles rather than reproduce any one health board's local operating policy.
Welsh Government's proactive-care framework emphasises adults with complex needs and older people living with frailty in the community, prevention, resilience, earlier intervention and coordination across health and social care. The centre will retain a UK-wide educational stance while explicitly signposting Wales-specific frameworks where useful.
