Designing an older-person & frailty pathway

Connect identification, urgent response, CGA, rehabilitation, community care and palliative support around a longitudinal person-centred pathway.

Design around transitions, not organisations

The person experiences one journey even when the system is divided into primary care, ambulance, emergency medicine, acute wards, community hospitals, therapies, social care, care homes and palliative services. A good pathway makes those transitions clinically coherent.

Pathway design questions

  • How is frailty identified and recorded across settings?
  • Who can provide same-day assessment and senior clinical decision-making?
  • Where can CGA begin and how does it follow the person?
  • How are rehabilitation and Home First principles protected?
  • What are the safe criteria for community management, step-up care and transfer?
  • How are care homes and carers linked into the MDT?
  • How are palliative and end-of-life needs identified early rather than only at crisis?
  • Which outcomes matter: function, independence, experience, continuity, place of care and avoidable utilisation?

Workforce capability

Current NHS England guidance explicitly recommends multi-agency frailty training across professions. ClinicalEdu's centre is designed to support that need with shared core knowledge and profession-specific routes rather than separate educational silos.
